Hypertrophic Cardiomyopathy Comparison of Dual-Chamber Pacing Versus Septal Myectomy for the Treatment of Patients With Hypertrophic Obstructive Cardiomyopathy A Comparison of Objective Hemodynamic and Exercise End Points

چکیده
RESULTS Baseline symptom status, left ventricular outflow tract gradients, exercise times and maximal oxygen consumption peak were similar between the two groups. Left ventricular outflow gradient was reduced from 76 6 57 to 9 6 17 mm Hg (p 5 0.0001) after myectomy, and from 77 6 61 to 55 6 39 mm Hg (p 5 0.07) after pacing (p 5 0.02 for comparison with myectomy). Ninety percent of myectomy patients experienced symptomatic improvement as compared with 47% in the pacing group. Exercise duration increased significantly from 6.6 6 2.8 to 8.7 6 3.0 min (p 5 0.0003) after myectomy compared with a change from 6.4 6 2.1 to 7.0 6 2.2 min (p 5 NS) in the pacing group. Maximal oxygen consumption increased from 19.4 6 6.4 to 22.2 6 6.5 ml/kg/min after myectomy (p 5 0.004), whereas the pacing group did not experience any significant change (19.6 6 6.5 vs. 20.1 6 6.5 ml/kg/min, p 5 NS).

Impact of dual-chamber permanent pacing in patients with obstructive hypertrophic cardiomyopathy with symptoms refractory to verapamil and beta-adrenergic blocker therapy.
BACKGROUND Patients with obstructive hypertrophic cardiomyopathy (HCM) with symptoms refractory to drugs (beta-blockers or verapamil) are candidates for cardiac surgery (left ventricular septal myectomy or mitral valve replacement). The present study examines prospectively the ability of dual-chamber (DDD) pacing to improve symptoms and relieve left ventricular outflow obstruction in such patie...

متن کاملReversible ischaemia in hypertrophic cardiomyopathy.
Atypical and typical chest pains are common symptoms in patients with hypertrophic cardiomyopathy. Some of these chest pains seem to be caused by ischaemia. It is difficult to objectively demonstrate ischaemia in hypertrophic cardiomyopathy. The first line treatment for chest pain considered to be ischaemic in patients with hypertrophic cardiomyopathy is the use of either a beta blocker or calc...
متن کاملComparison of Long-Term Effect of Dual-Chamber Pacing and Alcohol Septal Ablation in Patients with Hypertrophic Obstructive Cardiomyopathy
INTRODUCTION Nonpharmacological treatment of patients with hypertrophic obstructive cardiomyopathy (HOCM) comprises surgical myectomy (SME), alcohol septal ablation (ASA), and dual-chamber (DDD) pacing. The aim of the study was to compare the long-term effect of DDD pacing and ASA in symptomatic HOCM patients. PATIENTS AND METHODS We evaluated retrospective data from three cardiocenters; ther...
متن کاملEvolving Treatment for Patients With Hypertrophic Obstructive Cardiomyopathy.
D ynamic left ventricular outflow obstruction is a major cause of exertional symptoms in patients with hypertrophic cardiomyopathy. The obstruction is secondary to systolic anterior motion of the mitral valve abutting against a hypertrophied septum, resulting in diastolic dysfunction, myocardial oxygen supply-demand mismatch, reduced forward output, and secondary mitral regurgitation. Medical t...
